In accordance with Texas law, clients have the right to request access to their health care records maintained by this Licensed Professional Counselor (LPC). Requests for records must be made electronically to the clinician.
Upon receipt of a valid request, records will be provided within the time frame required by Texas law. Reasonable fees, as permitted by law, may apply for copying or preparing records. In certain circumstances, information may be withheld, summarized, or provided through a treatment summary when allowed by law to protect client welfare or confidentiality.
